This section is to be completed by all clients receiving a massage, body wrap, skin care, and/or waxing service.
understand appointments must be cancelled or rescheduled at least 24-hours prior to the scheduled service. Cancellations or rescheduling within the 24-hour notice and “no shows” will result in the FULL SERVICE FEE charged to the card on file. I have answered all questions and supplied personal information honestly. I am in general good health and understand certain products, services, and techniques are unsafe for those with specific health concerns. I understand Essentials Massage, Facial, Hair, and Nails of Brandon does not recommend skin care services for guests using Retin-A, Accutane, or products containing Alpha Hydroxy (e.g. glycolic acid). I agree to release Essentials Massage, Facial, Hair, and Nails of Brandon from all liability in the event of any medical/health problem being experienced by me during or after any service provided by Essentials Massage, Facial, Hair, and Nails of Brandon. It is also my responsibility to inform Essentials Massage, Facial, Hair, and Nails and its team members of any future changes in my health and/or medications. All minors must have a parent present at the time of check-in to authorize any massages, facials, or waxing services.
